How to request an Early Intervention evaluation

A step-by-step script for calling your state program, with what to say and what to ask.

You can refer your own child. You do not need a doctor's note in most states.

Say: 'I would like to request an evaluation for my child's speech and language development. I would like the evaluation done in Haitian Creole and English.'

The evaluation is free. Ask for a written copy of the results in your preferred language.
